Authorising a person to enquire or act on your behalf for employment services (nominee arrangement) with the National Customer Service Line.

The Department of Education, Skills and Employment's nominee arrangement process will allow employment services participants to appoint an authorised nominee to speak and act on their behalf with the department’s National Customer Service Line (NCSL) in relation to employment services.

You can use this form to authorise a person to enquire or act on your behalf for employment services. This form sets out the appointment of the nominee, advice on job seekers’ personal information, details about what matters the nominee can discuss and guidance on cancelling the arrangement. 

To submit a nominee arrangement, or change an existing arrangement fill out the Authorising a person to enquire or act on your behalf for employment services form.  Then give it to the nominee to complete and sign their section of the form. You can email the completed and signed form to us at the nationalcustomerserviceline@dese.gov.au mailbox
